• Members receive a 20 page health report which provides positive reinforcement of good health practices aloing with recommendations for imporvement when needed.

Telephonic Optimal Health Coaching

• Outbound Health Coaching provided to 100% of members who complete the Health Risk Assessment all from the comfort of home.
• Coaches work with members to assist them in reducing modifiable health risks and improve their overall health status.
• Goal setting with members to address highest risk areas according to readiness to change.
• Integrated mail based and electronic intervention programs.
